Consent to Groom and General Liability Waiver

Onyva is committed to the safety and well-being of all pets, and we want to assure you that we will make every effort to make your pet’s grooming visit as pleasant as possible. This is why, in part, it is not our general practice to use restraints, muzzles, or kennels.

  1. Client understands that due to the nature of pet grooming all quoted prices prior to grooming Client’s pet are only estimates. Upon physical inspection of your pet by Onyva, a good faith estimate of the price will be provided. Final prices will depend upon the temperament of each pet, their tolerance of the grooming process, and the condition of the pet’s coat, which ultimately drives the time necessary to complete services and, thus, costs.
  2. Client understands that Onyva has the right to refuse service to Client and/or Client’s pet at any time for any reason.
  3. If during an appointment it becomes apparent that your pet is not able to be groomed due to aggression/behavior or any other issue, the Client will only be charged for the services have already been performed and an additional appointment shall be scheduled to complete the services.
  4. Injuries such as nicks from clippers, scissors, or toenail trimmers may result if your pet does not remain still during the grooming or toenail trimming procedures.Such injuries are always unfortunate, but are common in the industry.  Client understands the risks of potential injury to their pet as a natural part of the grooming procedure and will hold Onyva harmless and without liability in such circumstances.
  5. If your pet has any allergies, medical conditions, or known behavioral conditions (i.e., epilepsy/seizures, diabetes, arthritis, separation anxiety, asthma, etc.), it is extremely important that you make Onyva aware of these medical & behavioral issues prior to booking your service as well as prior to the performance of the service so we can be certain to take the necessary steps to ensure your pet’s utmost safety and comfort as well as the safety of our employees and the other pets being serviced by Onyva.
  6. Client assumes all liabilities, financial and otherwise, for the behavior and health of their pet. In consideration of the services rendered by Onyva, Client waives any and all claims, actions, or demands of any nature, foreseen or unforeseen relating to the care, control, and health of Client’s pet arising during or after services performed by Onyva. Client expressly agrees that under no circumstances shall Onyva be liable for consequential or punitive damages.
  7. Occasionally, grooming can aggravate a current, or expose a preexisting medical condition. Such medical conditions could arise during or after the grooming procedure. In the event of this situation, the Client shall not hold Onyva liable. Client shall also not hold Onyva liable for any skin irritations that may result from grooming procedures or products used, or any stressful effects that grooming may have on young, geriatric, timid, or temperamental pets.
  8. Onyva is committed to respecting and not stressing the dogs it services.When a dog is matted, the removal of the matts will take precedence over all other bathing and grooming services.  Onyva always strives to preserve the coat whenever possible.  Although more expedient, we avoid shaving down dogs’ coats as it is ultimately unhealthy for the pet.  Onyva limits de-matting sessions to sixty (60 minutes), as longer sessions tend to stress the animal.  If the time limit is reached and the dog still has matts, the session is ended, the dog is removed from the table, and an appointment is booked at Onyva for continued dematting services to be provided within forty-eight (48) hours (whenever possible).  Each de-matting session will be individually billed.  The Client shall not hold Onyva liable for any problems uncovered on a badly matted or otherwise neglected coat or integument.
  9. The Client states that she/he is the pet’s owner; that said pet has not been exposed to any infectious illness within the last thirty (30) days; that the pet has been properly inoculated for the following: Rabies, Distemper, Parvovirus, Leptospirosis, and Bordetella, as well as a negative fecal exam within the last year; and that the pet is currently and properly licensed. I further certify that the information provided by me herein is true and accurate and that I have read this entire agreement.

Client fully understands there are inherent and potential risks involved in interactions between humans and pets.  This may result in property damage or bodily injury, including, but not limited to, permanent disability, sickness etc. to human or pets.  There may be other risks not known nor readily foreseeable at this time.  Client further understands that Onyva staff may at times have to restrain Client’s pet. Should such become necessary for the pet’s safety, the safety of Onyva staff, and/or the safety of other Onyva clients’ pets, additional injury may occur.  Client fully accepts and assumes all risks and responsibilities, including, without limitation, all losses, costs, and damages incurred as a result of Client’s pets participation in grooming, including any veterinary expenses incurred on behalf of Client’s pet.
